29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 05/28/15 in the Exhaust Hanger Department; employer failed to ensure the operator of the horizontal Muffler Hanger Winder applied a lock on the disconnect switch and the main air prior to performing service and maintenance on this machine, such as, changing of the spool and associated needle threading, as described in portions of the Muffler Hanger Winder SF 052 Energy Control procedure dated 11/30/2009, exposing them to puncture and caught between hazards. b) On or about 05/28/15 in the Exhaust Hanger Department; employer failed to ensure the operator of the vertical Muffler Hanger Machine applied a lock on the Machine Disconnect Switch and main air supply prior to performing service and maintenance on this machine, such as, changing the spool of winding and the associated needle threading, as described in the Muffler Hanger Machine SF052 Energy Control procedure dated 10/26/12, exposing them to puncture and caught between hazards. c) On or about 05/28/15 in the Exhaust Hanger Department; employer failed to ensure the operator of the Muffler Hanger Cutter applied a lock on the local disconnect switch and main air supply prior to performing service and maintenance on this machine, such as, changing the cutting blades, as described in the Muffler Hanger Cutter SF052 Energy Control procedure dated 11/30/09, exposing them to laceration hazards.
