29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: On or about April 3, 2015, at 585 Jackson Lake Road, Monticello, Georgia, two employees, performing roofing work using an approximately 35-foot tall shingle ladder with the base set within three feet of an approximately 30-foot tall overhead power line of approximately 14,400 volts were electrocuted when the single ladder came in contact with the overhead power lines.
