29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Facility - On or about 3/11/15, the maintenance employee services and repairs, parts such as the air cylinders, on the facility equipment including but not limited to: the HD/VTS Trim Press/Serial # 1183 (480V/60Hz/3 phase, 110 psi). The employer does not have an established lockout tagout program for the control of energy, employee training and periodic inspections for employees required to perform and/or service machines or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury. ABATEMENT DOCUMENTATION REQUIRED
