North Coast Plastics, Inc., 2114 Loveland Avenue, Erie, PA 16506, on or about February 20, 2015 and times prior thereto: A forklift operator with prior experience was not evaluated, and trained on workplace specific conditions and hazards at the facility prior to operating the Hyster S40XL forklift truck to ensure that he was competent to operate the forklift.
