29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: (a) Amorphous Silica Area: Employees were issued a 3M brand half face elastomeric respirator with P100 cartridges for voluntary use while bagging Amorphous Silica. The employer did not identify and evaluate the respiratory hazards in the workplace for the Silica production area that had a steady increase in production resulting in an increase in dust exposure, as determined on or about 02/10/2015.
