Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) .....the hazard of being struck by an overhead door. Between Bulding &17 and #34, employees were exposed to a crushing injured due to an overhead door (automatic) that was not equipped with a safety sensing device to stop the door or return it to the up position if it strikes something during its downward travel. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Provide a sensing device in the door frame. 2. Install a positive pressure switch to activate the door. b) .....the hazard of being struck by an overhead door. At Building #73 on the 1st floor (storage area), employees were exposed to a crushing hazard due to the overhead door in use not having a safety sensing mechanism to return or stop the door if it were to strike something. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Provide a sensing device in the door frame. 2. Install a positive pressure switch to activate the door. c) .....the hazard of being struck by an overhead door. At the east and west truck lockout stations of Building #44, employees were exposed to a crushing hazard due to the overhead doors not having a safety sensing mechanism to return or stop the doors if they strike an object. 1. Provide a sesning device in the door frame. 2. Install a positive pressure switch to activate the door. d) .....the hazard of being struck by an overhead door. At Building #111 on the 1st floor, employees were exposed to a crushing hazard due to the overhead door not having a safety sensing mechanism to return or stop the door if it were to strike something. At Building #7 on the 1st floor, employees were exposed to a crushing hazard due to the overhead door in the maintenance area not having a safety sensing mechanism to return or stop the door if it were to strike an object. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Provide a sensing device in the door frame. 2. Install a positive pressure switch to activate the door. f) .....the hazard of being struck by an overhead door. At Building #134 on the 1st floor, employees were exposed to a crushing injury due to an automatic overhead door not having a safety sensing device or positive pressure switch to stop or return the door during its downward travel if it were to strike something. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Provide a sensing device in the door frame. 2. Install a positive pressure switch to activate the door. g) .....the hazard of being struck by an overhead door. In Building #34 on the sough side of palletizer B, the automatic door does not release when it comes in contact with an object when closing. Among other methods, one feasible and acceptable means of abate- ment is to: 1. Provide a sensing device in the door frame. 2. Install a positive pressure switch to activate the door. DISCLAIMER: 1.
