29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with signed reports, were not performed: (a) Byers Industries, Inc. at 810 S. Sherman St., Longmont, CO 80501: On and before January 14, 2015, the employer did not ensure that all hoist chains associated with the 1-Ton and 2-Ton cranes in the production area had signed monthly inspection reports. The condition exposed employees to the hazard of being struck by heavy loads.
