29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: a. Site- Employees working from the second floor were not protected from a fall of 11 feet where the wood guardrail system built around the perimeter of the floor was constructed using support posts spaced 12 feet on center, wood guardrail framing was connected with nails, and the support posts on either side of the ladder way were not framed in two directions to provide strength to the termination points of the guardrail system. On or about January 12, 2015 an employee sustained severe injuries after falling from the second floor at the ladder way where the guardrail system failed when the employee was leaning against the top rail. The guardrail connected together with nails pulled apart when a force less than 200 pounds was applied.
