29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. Location: 75 West End Avenue, New York, NY. Elevator Shaft. a) Supervisors did not enforce the utilization of energy control procedures by employees as they worked in the elevator shafts on the elevators and associated equipment without implementing lockout tagout procedures. On 1/9/15, an employee was working inside the elevator shaft, at the lobby level, Energy control procedures were not utilized on Elevators 7,8 and 9 when the employee worked in the shaft.
