29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) were not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) Production, grinding department - employees assigned to perform setup tasks in CNC creep feed grinding and CNC vertical milling machines had their finger(s) and hand(s) exposed to caught in-between and entanglement hazards capable of causing amputations where an orderly shutdown of the equipment was not carried out. (b) Production, grinding department - employees assigned to perform adjustments in CNC creep feed grinding and CNC vertical milling machines had their finger(s) and hand(s) exposed to caught in-between and entanglement hazards capable of causing amputations where an orderly shutdown of the equipment was not carried out. The configuration, programming and use of the access door interlock devices (alternative measures) did not provide effective protection. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
