29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: -On or about October 17, 2014 and prior to that time there was a damaged wire sling attached to the fuel tank which had bird caging over a one inch area of the sling exposing the inner core which had not been removed form service or tagged " Do Not Use" .
