29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Pump Room 2: On or about October 22, 2014 the employer did not develop, document and utilize procedures for the control of potentially hazardous energy. Employees were exposed to safety and health hazards while replacing piping without informing the onsite employer of their lockout tagout procedures.
