29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): Building 4, 72 Lockwood Street, Newark, NJ a) The employer failed to perform a hazard assessment for a mechanic performing live voltage testing on equipment using only safety glasses. During repair of the Pulva-Sizer Size B mill, an employee used a Fluke 77 Series II Multimeter to test 240 Volt three phase circuit conductors inside of the A-B Grinder Side B # 4067 switchbox to determine if the Techtool Premium 20 Horsepower motor was correctly powered. Violation occurred on or around 8-15-14. b) The employer failed to perform a hazard assessment for a mechanic performing live voltage testing on equipment using only safety glasses. When the mechanic tried to repair the Model Number A-2 and Serial Number 2056 Pulva-Sizer Size A mill, the worker used an Amprobe Ultra Amperage meter inside of the A-B switchbox to determine if the Pulva-Sizer 5 horsepower motor was correctly powered. Violation occurred on or around 9-15-14.
