29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) At the job site, on October 8, 2014, the employer did not ensure an employee was provided the proper means of access and egress from an excavation, that measured five feet in death with a spoil pile that measured three feet in height that was placed at the edge of the excavation, who was performing preparation work for the installation of force and reclaim sewage lines, exposing the employee to hazards associated with broken bones, sprains, bruises, and lacerations.
