29 CFR 1910.23(b)(1)(i): Rail, roller, picket fence, half door or equivalent barrier was not provided on the opening for handling materials, the guards were not kept in position regardless of a door on the opening. On or about October 1, 2014, employees were exposed to fall hazards when the double doors used to place paper records in the records room in building 950 could be opened with the key available in the storage room. There were no guardrails available once the doors were opened.
