29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allowed, the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed: On or about October 2, 2014, on the east side of the building, the aluminum extension ladder used by employees to access the work area did not have side rails extended at least three feet above the edge of the roof. Employees were exposed to falls of up to approximately 16 feet to the concrete below.
