29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a. Production Floor near beverage labeler- On or about 14 August 2014, employees were required to lock-out and tag-out labeler production line at 480V disconnect panel. There was no lock-out tag-out device attached to panel box to alert maintenance personnel and/or affected employees of the machines actual status. Thereby exposing employees to accidental start-ups which would result in cut's, lacerations, and amputations.
