29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On or about 06/24/2014 at the Locust Lane Re-Roofing Project, two employees were using extension ladders to access and descend to and from the roof and the ladder did not extend 3 feet past the landing nor were the ladders secured and a grasping device provided. Pro Quality Home Improvement LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 765201, citation number 02, item number 002 and was affirmed as a final order on 02/01/2013, with respect to a workplace located at 80 Stony Mill Lane in East Berlin, CT. Pro Quality Home Improvement LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312882244, citation number 01, item number 005 and was affirmed as a final order on 04/20/2009, with respect to a workplace located at Johnsons Point Road in Branford, CT.
