29 CFR 1915.152(b): The employer did not assess work activity to determine whether hazards are present, or likely to be present, which necessitate the employee's use of personal protective equipment (PPE): (a) USS Boxer (LHD-4), Flight Deck Area, Cargo and Weapon's Elevator: On July 31, 2014, the employer failed to conduct a hazard assessment to determine the existence of fall hazards. Employees were exposed to falls up to 98 feet while applying masking tape to the elevator platform without the use of a personal fall arrest system. (b) USS Boxer (LHD-4), Flight Deck Area, Cargo and Weapon's Elevator: On July 30, 2014, the employer failed to conduct a hazard assessment to determine the existence of fall hazards. Employees were exposed to falls up to 98 feet while applying primer paint to the elevator platform without the use of a personal fall arrest system. (c) USS Boxer (LHD-4), Flight Deck Area, Cargo and Weapon's Elevator: On July 29, 2014, the employer failed to conduct a hazard assessment to determine the existence of fall hazards. Employees were exposed to falls up to 98 feet while removing non-skid paint on the elevator platform without the use of a personal fall arrest system. (d) USS Boxer (LHD-4), Flight Deck Area, Cargo and Weapon's Elevator: On July 22, 2014 the employer failed to conduct a hazard assessment to determine the existence of fall hazards. Employees were exposed to falls up to 98 feet while installing plastic containment inside the elevator platform without the use of a personal fall arrest system.
