29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: The employer does not ensure a lock and a tag are placed on each disconnecting means used to de-energize circuits and equipment on which work is to be performed. a. This violation occurred on or about August 4, 2014, in the Lung Pump Room where employees were exposed to approximately 289 VAC electrical hazards when replacing Scalder Blower motor #5 without a lock and a tag being placed on the electrical disconnects in Motor Control Center Room 1. b. This violation occurred on or about August 4, 2014, in the Lung Pump Room where employees were exposed to approximately 289 VAC electrical hazards when replacing Scalder Blower motor #6 without a lock and a tag being placed on the electrical disconnects in Motor Control Center Room 1. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a lock and a tag are placed on each disconnecting means used to de-energize circuits and equipment on which work is to be performed.
