29 CFR 1910.23(d)(1): Flights of stairs having four or more risers were not equipped with a standard railing as specified in paragraphs (d)(1)(i) through (v) of this section: A) LWCF building, on or about August 5, 2014, the fixed stairs with 15 risers did not have a mid-rail as part of the stair-rail system exposing employees to a fall hazard. B) Power house manager's Office on or about September 23, 2014, the fixed stairs with 6 risers did not have a mid-rail as part of the stair-rail system exposing employees to a fall hazard. C) #3 Recovery Boiler FD Fan north side 1st floor, on or about September 23, 2014, the fixed stairs with 5 risers did not have a mid-rail as part of the stair-rail system exposing employees to a fall hazard.
