29 CFR 1926.1419(f): During operations requiring signals, the ability to transmit signals between the operator and signal person were not maintained. When that ability is interrupted at any time, the operator did not safely stop operations requiring signals until it is reestablished and a proper signal is given and understood: On or about July 21, 2014, at the above addressed jobsite, employees were exposed to being struck-by a joist which was suspended by a crane, while the crane's signal man was using both hands to guide the joist and was unable to use the radio to transmit signals to the crane operator.
