29 CFR 1926.1101(d)(5): As supervisor of the entire project, the general contractor did not ascertain whether the asbestos contractor was in compliance with the asbestos standard, and did not require such contractor to come into compliance with this standard: a) On or about 20 June 2014, the employer, acting as a General Contractor, requested, directed and approved of a sub-contractor employee to cut two pipes with asbestos thermal system insulation (TSI) elbows containing 5-35% chrysotile asbestos in the cafeteria, thereby exposing employees to the hazards associated with asbestos and did not require the sub-contractors to come into compliance this standard. 29 CFR 1926.405(j)(1)(i): Fixtures, lampholders, lamps, rosettes, or receptacles had live parts exposed to employee contact: a) On or about 23 June 2014, the employer used office electrical equipment plugged into an outlet receptacle which was not attached to the outlet box and did not have a faceplate, which exposed live parts.
