29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator had been involved in an accident or near-miss incident On or about May 19, 2014, an employee was involved in an accident with a powered industrial vehicle, where the employee struck and bent a rack beam with the forks of a forklift, exposing the employee to falling materials. The employer did not provide the operator involved in the accident with refresher training. There is no abatement certification or documentation required for this item.
