29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about July 19, 2011 employees engaged in prep work operations for concrete roadway pour in extreme temperatures were exposed to heat stress hazards. The employer did not provide training and instruction to employees regarding hazards and unsafe conditions including, but not limited to, informing employees about the effects of heat stress, and how to recognize heat related symptoms and prevent heat-induced illnesses. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
