29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. On or about April 16, 2014, and times prior thereto, in the stock room/receiving area, boxes of merchandise was stacked and stored approximately 12 feet to 15 feet in height in an unstable manner, exposing employees to being struck-by falling boxes. The Dollar Tree Stores, Inc. was previously cited by the Providence Area Office for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.176(b), which was contained in OSHA inspection number 951336, citation number 1, item number 004, issued on December 19,2013 and was affirmed as a final order on January 16, 2014, with a final abatement date of December 24, 2013, with respect to a workplace located at 1400 Oaklawn Avenue, Cranston, RI. 02920.
