29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: a. On or about April 11, 2014, on the front of the house, the roof access ladder did not extend at least 3 feet above the eave, exposing employees to an 11 feet fall hazard.
