29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Along the southeast corner of the construction building, on or about April 09, 2012, an employee was working inside a trench approximately 5 1/2 feet deep by 2 feet wide by 52 feet long without any type of adequate cave-in protection.
