29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about March 12, 2014 and prior thereto, employees at the back of Mill #3 were exposed to the hazard of being caught between the bridle and the plumbing for the hydraulic system when the bridle was being opened or lowered. b. On or about March 12, 2014 and prior thereto, employees at the back of Mill #2 were exposed to the hazard of being caught between the bridle and the plumbing for the hydraulic system when the bridle was being opened or lowered. c. On or about May 14, 2014 and prior thereto, employees at Mill #2 starting the metal on the rewind core were exposed to the hazard of their hands being caught between the core and the ironing roll when the ironing roll was lowered. d. On or about May 14, 2014 and prior thereto, employees at the back of Mill #3 walking and working near the payoff were exposed to the hazard of contacting the coil of rotating metal and being cut. e. On or about May 14, 2014 and prior thereto, employees at the front of Mill #3 walking and working near the rewind were exposed to the hazard of contacting the coil of rotating metal and being cut. f. On or about May 14, 2014 and prior thereto, employees at the back of Mill #2 walking and working near the payoff were exposed to the hazard of contacting the coil of rotating metal and being cut. g. On or about May 14, 2014 and prior thereto, employees at the front of Mill #2 walking and working near the rewind were exposed to the hazard of contacting the coil of rotating metal and being cut. h. On or about May 14, 2014 and prior thereto, employees at the back of Mill #1 walking and working near the payoff were exposed to the hazard of contacting the coil of rotating metal and being cut. i. On or about May 14, 2014 and prior thereto, employees at the front of Mill #1 walking and working near the rewind were exposed to the hazard of contacting the coil of rotating metal and being cut. j. On or about May 14, 2014 and prior thereto, employees at Mill #3 walking between the operators control panel and bridle control and the plumbing for the hydraulics on the back side of the mill to observe the metal going from the payoff to the mill and breaking the metal with a knife or stick were exposed to contacting the moving metal and being cut. k. On or about May 14, 2014 and prior thereto, employees at Mill #2 walking between the operators control panel and bridle control and the plumbing for the hydraulics on the back side of the mill to observe the metal going from the payoff to the mill and breaking the metal with a knife or stick were exposed to contacting the moving metal and being cut.
