29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent: a) On or about, March 4, 2014, and at times prior thereto, the in ground vehicle lift in bay #7 was not tagged out at the controls to indicate that it was not fully operable. b) On or about, March 4, 2014, and at times prior thereto, two brake lathes in the store room were not tagged out to indicate they were defective or not in service.
