29 CFR 1910.119(d)(3)(i)(B): Information pertaining to the equipment in the process did not include piping and instrument diagrams (P&ID's): At the facility, the employer had not verified that the p&id's were up to date and accurate for use by Milliard Refrigeration Services employee(s) and contractors for line breaks, equipment / piping replacement or repairs, and etc. Several piping circuits were evaluated and the following were noted: (a) Drawing ME-A-P-0512 (document #8) during the walk around a check valve and a globe valve that had a pad lock & chain attached to it on the ammonia charge line, were not drawn on the p&id. (b) Drawing ME-A-P-0512 (document #8) process piping 3"-RV-9027-XR was drawn on the p&id, but not observed during the walk around. (c) Drawing ME-A-P-0512 (document #8) process piping 3"-G-223 was drawn on the p&id, but not observed during the walk around. (d) Drawing ME-A-P-0512 (document #8) an unidentified pipe line connected process pipe lines 3"-L-222 and 1"-RV-9026-XR was observed on the walk around, but not drawn on the p&id. (e) Drawing ME-A-P-0513 (document #8): during the walk around an angle globe valve was observed on pipe line 1 1/2"-G-246 and a globe valve was observed on pipe line 1 1/2-L-1990, but not drawn on the p&id correctly. The p&id had the globe valve drawn on the 1 1/2-G-246 and the angle globe valve was drawn on the 1 1/2-L-1990. Employee(s) were exposed to release of ammonia vapor / liquid to the atmosphere that historically results in health hazards leading to injury and death to persons in the workplace.
