29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation: On or about January 29, 2014, Enbridge Pipelines (Texas Gathering, L.P.) exposed employees in the process of changing valves on an existing pipeline to fire/explosion hazards. The employer did not ensure that flammable hydrocarbons did not re-accumulate during maintenance activities.
