29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): a) for employees exposed to a cave-in hazard while working inside an excavation located at 4300 S Frontage Road in Lakeland (FL), as observed on or about 01/03/2014. The south trench wall was benched at 76 degrees and the north trench wall was benched at 88 degrees. The B.R.W. Contracting Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 316355502, citation number 1, item number 1 and was affirmed as a final order on 04/23/2012, with respect to a workplace located at Family Dollar in Tampa Florida.
