29 CFR 1910.27(f): Ladders were not maintained in a safe condition or inspected regularly: On or about November 07, 2013, and at times prior thereto, employees were working on a rig site. The ladder to access the derrick had a rung that was bent extensively as to project out and cause a slip/trip hazard. Employees were exposed to falls of 4 feet or more.
