Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by or caught between a Jacobsen AR-5 Contour Rotary Mower: This violation was discovered on or about November 20, 2013, and at times prior thereto, where the employer did not ensure that a Jacobsen AR-5 Contour Rotary Mower, marked GM-1, had the manufacturer's required seatbelts installed.
