29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about October 7, 2013, lockout or tagout devices were not affixed to the control panel for the conveyor systems located in the South Plant metal room/ north. An employee was tasked with having to perform maintenance on the Mayfran B4 conveyor system (serial number- c7265-9 mayfran 930651).
