29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Paint booth and yard area, on or about September 10, 2013, the employer did not ensure that employees who worked at heights 4 feet and greater while performing work on top of locomotive trains were provided with fall protection such as, but not limited to personnel fall arrest system, restraint system, or work platform with standard guardrail system attached, thus exposing employees to struck-by and fall hazards up to 15 feet.
