29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. a. On or about July 22, 2011, in the Raque Room, an employee was replacing the rusty bolts on the filling dispenser head and creme dispenser head and did not lock out the pneumatic energy source. b. In the Back Packaging area on the Shanklin Overwrap Machine an employee was cleaning the rubber base on the table where the overwrap knife strikes to cut the wrap and did not lock out the electrical and pneumatic energy sources. c. In the Back Packaging area on the Taping Machine an employee replaced a tape roll and did not lock out the electrical energy sources.
