29 CFR 1910.119(e)(5): The employer did not establish a system to promptly address the teams findings and recommendations: a) Enterprise Products Operating, LP at 27991 County Road 5, Rifle, CO 81650: On and before 7/30/13, the employer did not establish a system to promptly address the teams findings and recommendations in the process hazard analysis (PHA) in a timely manner or that the resolution was documented. Findings include but are not limited to: 1) The employer failed to modify or relocate the Control room building to lower the maximum predicted BLD below 2 or relocate personnel. 2) The employer failed to update the facility procedures and ensure all personnel are trained and proficient in PSM standards Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
