29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address engineering and administrative controls applicable to the hazards: The employers 9/19/2008 process hazard analysis identified the following hazards without completing appropriate methodology to address engineering and administrative controls on the PHAWorks format in the form of safeguards, risk assessment values, layers of protection analysis, recommendations, and comments: (a) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 3 - Manual block valve inadvertently closed on one exchanger. (b) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 6 - All furnace passes fail open. (c) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 8 - Loss of flow through an exchanger. (d) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 11 - Exchanger inadvertently bypassed. (e) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 16 - Loss of steam. (f) Node 6 - Primary Column Bottoms Heat Exchange Train, Cause 17 - Loss of electricity.
