29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a) Chains and four sprockets powering the rollers on the Brewer notcher conveyor systems were not guarded to prevent employee exposure to the pinch points, resulting in multiple amputation injuries. b) Three chains and six sprockets on the back of the Brewer notcher were not guarded to prevent employee exposure to the pinch points hazards, that could result in an amputation. c) A chain and sprocket were not guarded on the trim table where the operator feeds lumber into the saw blades, creating a pinch point hazard, that could result in an amputation.
