Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.212 (a)(1): The employer did not provide one or more methods of machine guarding to protect the operator and the other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts: (a) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not provide one or more methods of machine guarding to protect the operator and the other employees from the 2'' opening on the rotating shaft of the water pump. By not guarding the rotating parts on the water pump, employees were exposed to laceration hazards. (b) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not provide one or more methods of machine guarding to protect the operator and the other employees from the 2'' opening on the rotating shaft of the new portable chemical transfer pump from the blender to the mud tanks. By not guarding the rotating parts on the chemical transfer pump, employees were exposed to laceration hazards. (c) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not provide one or more methods of guarding to protect the operator and the other employees from ingoing nip points on the orange pneumatic, air winch, Model XJFH-5/35. This winch is positioned at the top of the steps leading to the rig floor and is approximately 4 feet off the rig floor. This condition exposed employees to caught-in hazards.
