Performance Castings, Inc., 242 East 16th Street, Erie, PA 16503, roof of the structure, on or about July 19, 2013 and times prior thereto: Employees were performing roofing activities at the peak of the roof without a means or method of fall protection when exposed to a fall hazard of 21.4 feet.
