29 CFR 1910.253(b)(5)(iii)(I): Fuel-gas was used from cylinders through torches or other devices equipped with shutoff valves without reducing the pressure through a suitable regulator attached to the cylinder valve or manifold: (a) In the maintenance shop, a Harris regulator for the oxygen cylinder on the Miller Big Blue welder was defective and was not operating properly, therefore it did not ensure suitable pressure regulation of the oxygen to the torch. The regulator indicated pressure when the valve was closed.
