Text For Citation: 01 Item/Group: 002 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards of more than 4 feet due to the following: a) Rig #4: On or about June 27, 2013 the employer did not ensure that stairways used by employees to access to and from the rig floor were secured from being displaced/dislocated. Pins or other suitable methods were not employed to secure the stairway. b) Rig #4: On or about June 27, 2013, the employer did not ensure that the escape route was free from obstructions. A wire rope that intersected with the Geronimo line exposed employees to hazards such as, but not limited to, falls.
