29 CFR 1910.23(a)(8)(ii): A floor hole cover of standard strength and construction was not in place, the floor hole was not constantly attended by someone or was not protected by a removable standard railing: (a) In the Mill at the DA edger, On June 24, 2013, and at times prior thereto, employees were exposed to a fall hazard when walking/ working at the NE end of the edger where the sawdust hole was open and un-protected.
