29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: (a) In the machines shop, at Moore Fabrication, located in Houston, Texas, on or about June 27, 2013 - Employees used an Ohio Forge 6" Bench Grinder (MN 59510 SN 05797) to help in their machine work which was not equipped with work rest, exposing employees to struck-by hazards.
