29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. a) 150 Farmington Lane, Lancaster Pa - An employee was observed using a portable extension ladder to gain access to a ladder jack scaffold and the ladder extended 18 inches above the landing surface exposing the employee to a fall hazard, on or about June 21, 2013. No abatement certification or documentation required.
