29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. a) Ground floor, metal formwork deck for concrete pouring: An employee wore a safety harness and anchored his lanyard strap around a shoring beam of the metal formwork. The shoring beam was not designed to support to support and be used as an anchor for a personal fall arrest system. The system was not designed to support 5,000 pounds per employee attached to the beam. On or about 05/21/13. b) Ground floor, metal formwork deck for concrete pouring: Employees tied off to straps that were nailed to the metal formwork deck as fall arrest anchorages. The panels of formwork deck, which were used as anchorages for attachment of personal fall arrest equipment, were not designed to support 5,000 pounds per employee attached. The strpas were not installed per the manufacturers design and specs and anchorage requirements. On or about 05/21/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19.
