29 CFR 1910.147(c)(6)(i): The employer did not conduct periodic inspections of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: A. Plasto-Tech Corporation worksite located at 708 Lowell St., Elyria, Ohio: On or about May 24, 2013, the employer did not conduct periodic audits of its energy control procedures to assure that proper lockout/tagout procedures and requirements were being followed. Deficiencies in procedures and the lack of procedure audits exposed employees to crush injuries in that adequate machine isolation was not being followed when employees accessed hazard areas of the Brown Thermo-Forming press while producing prototype parts.
